What you will do in your role

Health Homes Care Managers must know the social service and health care systems and be able to build relationships with families and partners. Applicants must be comfortable making home visits to clients’ homes, working closely with schools, health providers, social services, community service providers, etc. Additional duties include completing assessments, facilitating team meetings, writing plans of care, documenting all activities, referring clients to needed services, etc.

Qualifications

  • Minimum Associate's Degree in Human Services or related field from an accredited college or university AND ample proven experience as a case manager required.
  • Bachelor's Degree in Human Services or related field from an accredited college or University and two (2) years' related experience. (Foster Care, Child Welfare, Counseling, etc.) preferred.
  • Must have valid Driver's License and ability to maintain insurability throughout employment.

Job Type: Full-time

Pay: $19.00 - $23.00 per hour
